Partner Providers
Organizations and professionals who deliver eligible CalAIM services, such as Enhanced Care Management, Community Health Worker Services, and Community Supports, through a subcontract with TPF. Partner Providers complete TPF’s readiness, training, and onboarding through our Partner Provider Training Academy.
MOU with TPF
Community Partners
Schools, faith communities, community-based organizations, libraries, housing communities, and agencies that host programs, share resources, and create warm-handoff pathways. Community Partners work with TPF under an MOU (memorandum of understanding).

Haven for Birth & Refuge
Haven for Birth & Refuge provides maternal health and birth-equity services throughout pregnancy, childbirth, and postpartum recovery. Its services include doula care, birth education, labor support, lactation support, wellness services, family education, advocacy, and maternal healthcare navigation.
Maryvale
Maryvale supports children, youth, and families through trauma-informed early childhood education, behavioral health services, substance use prevention and counseling, Enhanced Care Management, family support, and temporary housing for women and children.
Maternal Mental Health NOW
Maternal Mental Health NOW works to reduce barriers to the prevention, identification, and treatment of perinatal mood and anxiety disorders. Its services include bilingual perinatal peer support, mental health education, individual peer connection, resource navigation, provider training, and culturally affirming support for underserved birthing communities.
Measure of Hearts Foundation
Measure of Hearts Foundation supports people experiencing homelessness, housing instability, justice involvement, and barriers to long-term stability. Its services include transitional housing, housing navigation, Community Health Worker Services, case management, Medi-Cal navigation, employment assistance, reentry support, and record-clearing assistance.
