Care that meets people where they are
TPF provides Community Care services to eligible individuals and families through contracted healthcare partnerships and community referral pathways. We bring care into the community rather than asking the community to come to care.
Our Care Model
TPF's Community Care model is built around our interdisciplinary Support Squad: licensed and Board of Behavioral Sciences-registered clinical professionals working alongside Community Health Workers and Lead Care Managers to provide whole-person care coordination. We believe that care is most effective when it is delivered by people who understand the community, speak its language, and are trusted within it. Our Support Squad model brings clinical expertise and community knowledge together in every interaction. When ongoing behavioral health treatment is needed, participants are connected to appropriate long-term providers within the community rather than being left to navigate that transition alone.
Who Is Eligible
Community Care eligibility may include Medi-Cal enrollment, Managed Care Plan eligibility, CalAIM eligibility, or referral from an approved community partner. If you are not sure whether you or someone you know is eligible, contact us and we will help you find the right pathway.
